The State of California has now stated that all new students entering school must show written verification of all immunizations. You may no longer just fill out the Blue Immunization card and sign it yourself. YOU MUST BRING IN TO THE OFFICE YOUR WRITTEN RECORD OF IMMUNIZATIONS SIGNED BY YOUR PHYSICIAN. FAILURE TO DO SO WILL MEAN YOUR CHILD WILL NOT BE ADMITTED TO CLASS IN SEPTEMBER.
Also, please make sure your child's immunizations are up-to-date.

Current Mantoux TB test with written verification for Kindergarten
students and new students entering L.A. County
Polio – 4 doses (3 doses if one given after 4th birthday)
DPT – 5 doses (4 if one given after 4th birthday)
MMR – 2 doses on or after 1st birthday)
Hepatitis B – 3 doses or new 2 dose regime
Varicella (Chicken pox) – 1 dose or immunity (had disease)

You may obtain immunizations at the Health Department, 330 W. Maple, Monrovia.
In order to complete our files, please make sure all forms have been completed and turned in to the office by August 31.
